DC residents (18 years of age and older) can apply for a medical cannabis card using the DC ABCA Adult Medical Cannabis Patient Application. If you are at least 21 years of age you may self-certify your application in lieu of receiving a healthcare provider recommendation. Online applicants can select the self-certification option under “Certification Type”. If you are between 18-20 years of age you are not permitted to self-certify and must include a healthcare recommendation number on your application.

For a temporary patient registration card, non-DC residents must submit a brief online registration form self-certifying that they will only use purchased medical cannabis for valid medical purposes.
